The Care Guide is responsible for delivering high-quality housing stability and care navigation services to health plan members. Serving as a trusted resource, the Care Guide conducts comprehensive assessments, develops individualized care plans, and connects members to community resources that address housing instability and related social determinants of health.
This role requires empathy, resourcefulness, and a deep understanding of the barriers faced by individuals experiencing housing instability. The Care Guide works within an interdisciplinary team to ensure coordinated, whole-person care that supports long-term member stability and well-being.
